 The FEDERATION OF SMALL AND MEDIUM INDUSTRIES (FOSMI) is an organization devoted to the welfare of Micro, Small and Medium Enterprises.  FOSMI has representation on many governmental and non-Governmental bodies and uses these platforms to further cause of MSME’s

 FOSMI Instituted it’s annual programme “AWARDS OF EXCELLENCE” so that the spirit, dedication and achievements of small entrepreneurs get the recognition they rightly deserve.  The first awards were presented in 1996 and since then have been an annual feature.

 FOSMI’s 10^th ‘AWARDS OF EXCELLENCE’ programme is scheduled to be held sometime in January 2008.  Awards would be given away to selected entrepreneurs in the following seven categories :

 1.  Outstanding Industrial Enterprise.                    2. Outstanding Export Performance 

3.  Outstanding Woman Entrepreneur.            4.Outstanding First Generation Entrepreneur

5.Outstanding Achievement in the Service Sector including Information Technology 

6. Outstanding performance of Self-help Group engaged in promotion of manufacturing / service activities   7.  Outstanding performance in Environment Protection

8.Outstanding performance in Khadi & Village Industries Commission / Khadi & Village Industries Board unit.

 ELIGIBILITY

• All Small Scale Industrial Units having commenced operations prior to 01-04-2004 are eligible to apply for the awards.  (The date is relaxable for I.T. Units)

• A unit competing in the “Woman Entrepreneur” Category should be at least be 60% owned by Woman.  The Chief Executive should necessarily be a woman.

 SELECTION 

 A committee comprising representatives of Government and Semi-Governmental organizations Financial Institutions, Banks, Promotional Bodies and FOSMI members will make the final selection of award winners.

 INSTRUCTIONS

• Forms should be submitted in the Prescribed Format or a Xerox copy of the same (to be available from DIC/FOSMI office)

• Please use extra sheets wherever necessary.
• Forms should preferable be typewritten.  For handwritten forms please use block capitals.
• Please attach one passport size photograph of the Proprietor/ Partner/ Director of the applicant unit/ secretary/ Prsident of SHG Group.
• Please attach photocopies of Awards/Certificates received.
• Please attach leaflets/catalogues of your products.
• Completed forms with attachments should reach FOSMI office by December 31, 2007
• Name of the recommending jauthority, if any, should be mentioned.
• For category 5 “Outstanding avhievement in the Service Sector including Information Technology” only Schedule Services as per guidelines of the Development Commissioner MSME, Govt. of India would be considered.
